Switching to a new dental office should not have to be a stressful process. We will do everything in our power to make your first dental appointment as straightforward as possible. Before you call and schedule your next appointment, review the information below so that you’re prepared in advance to the fullest extent possible.
Your First Dental Visit
Once you’ve been greeted by our kind front desk staff members, you’ll be given a number of new patient forms to fill out. You’ll also need to provide your dental insurance information so that we can file claims as needed. Once we call you back to our treatment area, we’ll complete a full examination of your mouth, which will include X-rays and a comprehensive checkup of the teeth and gums. We’ll explain if we find anything wrong and offer appropriate treatment options.
New Patient Forms
While you can fill out the required forms in our waiting room, we strongly suggest that you complete them before you get to our dental office if possible in order to streamline the check-in process. Print out the forms using the link below and bring them with you on the day of your appointment!
