Once you’ve been greeted by our kind front desk staff members, you’ll be given a number of new patient forms to fill out. You’ll also need to provide your dental insurance information so that we can file claims as needed. Once we call you back to our treatment area, we’ll complete a full examination of your mouth, which will include X-rays and a comprehensive checkup of the teeth and gums. We’ll explain if we find anything wrong and offer appropriate treatment options.
While you can fill out the required forms in our waiting room, we strongly suggest that you complete them before you get to our dental office if possible in order to streamline the check-in process. Print out the forms using the link below and bring them with you on the day of your appointment!
We’re happy to accept your dental insurance plans, including various PPOs that ensure that you’ll be able to afford the services you need to protect your smile. Take full advantage of your dental insurance benefits by scheduling two dental appointments a year and seeking dental care as soon as you need it.
